Code of Conduct Agreement Player & Parent Code of Conduct

Please read carefully and acknowledge agreement below.

We are committed to creating a positive, safe, and respectful environment for all players, families, coaches, and officials. By registering for this team, we agree to the following:

Player Expectations:

Demonstrate good sportsmanship at all times.

Show respect toward coaches, teammates, opponents, referees, and spectators.

Follow all team rules and coaching instructions.

Attend practices and games on time and ready to participate.

Refrain from bullying, fighting, or inappropriate language.

Take care of team equipment and facilities.

Parent/Guardian Expectations:

Support all players and coaches in a positive manner.

Respect coaches’ decisions and refrain from coaching from the sidelines.

Treat referees and opposing teams with respect.

Address concerns appropriately and at the proper time (not during games).

Promote good sportsmanship and team unity.

Ensure timely arrival for practices and games.

Failure to follow the Code of Conduct may result in disciplinary action, including reduced playing time or removal from the team.

Liability Waiver & Assumption of Risk

Participation in football involves inherent risks, including but not limited to serious bodily injury, permanent disability, paralysis, concussion, and death. By registering my child for participation, I acknowledge and fully understand the nature of these risks.

In consideration of my child being permitted to participate in team activities, including practices, games, training sessions, travel, and related events, I hereby voluntarily assume all risks associated with such participation.

I, on behalf of myself, my child, and our heirs, assigns, and legal representatives, hereby release, waive, discharge, and hold harmless the team, its coaches, volunteers, organizers, sponsors, league officials, and affiliated facilities from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by my child as a result of participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I certify that my child is physically fit to participate in football activities and that I have disclosed all relevant medical information in this registration form.

I further authorize team representatives to obtain emergency medical treatment for my child if necessary and agree to be responsible for any resulting medical expenses.
